Senior Pet Release Form

Please fill out the following form
in order to be seen by a groomer if your dog is 8 years or older.

I hearby grant permission to VIP Mobile Grooming (Very Important Pooch, LLC) to obtain emergency veterinary treatment for my pet. Also, realizing that Senior Pets have a greater chance of injury during grooming, I wll not hold this Grooming Establishment responsible for accident or injury to my pet during the grooming procedure.
By signing this release form, you acknowledge that you fully understand the risk of grooming a senior dog and are allowing Very Important Pooch (VIP Mobile Grooming) to perform this service, while releasing Very Important Pooch LLC (VIP Mobile Grooming) from any injury, loss, liability, claims, damages,and/or otherwise which may arise directly or indirectly as a result of the services requested.
